Ibogaine safety and cardiac risk
Ibogaine is not a low-risk substance. Nearly every published fatality associated with it traces back to a recognisable failure: no cardiac screening, no monitoring, undisclosed polysubstance use, or dosing in a non-medical setting. Understanding those failure modes is the difference between an informed decision and a dangerous one.
Why the heart is the central concern
Ibogaine and its metabolite noribogaine block the hERG potassium channel, which delays cardiac repolarisation and prolongs the QT interval. A prolonged QT interval raises the risk of torsades de pointes, a potentially fatal arrhythmia.
The effect is dose-dependent and, critically, long-lived: noribogaine has a long half-life, so QT prolongation can persist for days after a single session. Monitoring that stops when the psychoactive phase ends stops too early.
Absolute and relative contraindications
- Baseline QTc above roughly 450 ms (men) or 470 ms (women)
- Personal or family history of arrhythmia, long QT syndrome, or sudden cardiac death
- Structural heart disease, recent myocardial infarction, or heart failure
- Uncontrolled hypertension
- Severe hepatic or renal impairment (ibogaine is CYP2D6-metabolised)
- Electrolyte disturbance — low potassium or magnesium must be corrected first
- Concurrent QT-prolonging medication, including many antipsychotics, antiarrhythmics, macrolides and methadone
- Pregnancy, and active respiratory infection or seizure disorder
A defensible screening protocol
- 12-lead ECG with QTc calculated by a clinician, not an algorithm alone
- Echocardiogram where any structural concern exists
- Full metabolic panel with liver and renal function and electrolytes
- CYP2D6 consideration for poor metabolisers, who reach higher plasma levels
- Toxicology screen, with a mandatory washout for stimulants and long-acting opioids
- Psychiatric review, including psychosis and suicidality screening
Monitoring standards during and after dosing
The standard of care is continuous telemetry ECG from before dosing until QTc returns toward baseline, with a physician physically present, resuscitation equipment on site, IV magnesium available, and a defined hospital transfer plan.

How dangerous is ibogaine?
Published case series place the fatality risk in unsupervised settings far above that of supervised programs with full cardiac screening and continuous telemetry. Risk is dominated by screening quality and monitoring, not by the molecule alone.
How long does QT prolongation last?
It typically peaks within hours of dosing and can remain measurably elevated for several days because of noribogaine's long half-life.
Can ibogaine be taken with methadone?
No. Methadone is itself QT-prolonging and long-acting. Protocols require a supervised transition to short-acting opioids well before any dosing is considered.
